The movie has a huge theme of famine, many of the characters become violent in result of starvation so they may get food. Many people, children, and babies freeze to death or are sold.
A very violent movie on the evils of war, profiteering and consequences.

Mild amount of profanity
The American uses the Fuck word moderately often as is their wont, other obsenities are lightly said.
A Chinese woman uses a particularly nasty religious curse "fuck your ancestors".
Otherwise the Chinese equivalent is rarely used. (Not to say that Chinese people are less inclined to use bad language).

Some smoking, though given the time it was normal to smoke. Besides when food is in demand, smokes whether tobacco, opium or cannabis is lower priority.

The movie contains many scenes of war, particularly involving innocent people (collateral damage) but some actual soldiers. These often result in very bloody results and somewhat realistic.
There are many of the characters degrade themselves as the movie progresses. The rich become more corrupt than usual and the poor start doing things they wouldn't imagine they would do. The movie is very emotional and quite shocking if honest.
Depressing atmosphere throughout.
