The narrator describes how she's being assaulted by the men by saying they're "performing the old in out, and then another, and another". Multiple men are shown thrusting on top of her.

The main character Alex is a very brutal, psychotic and sadistic person who revels in the acts of violence he inflicts on innocents, which can be frightening and disturbing for some.

Throughout the film there are many frightening and intense scenes this includes a scene where Alex's eyes are clamped open and a part where deltoid spits in Alex's face.

The whole film has a very disturbing and almost surreal atmosphere throughout. While the violence is never graphic, it is still unsettling and many scenes are not comfortable to watch.
